Office Hours & Appointments

Our office is open from 8:00.m. to 4:30 p.m. Monday through Friday except holidays. We are closed for lunch daily from 12:00 noon to 1 p.m. If you wish to schedule, confirm, or change an appointment, call (434) 984-4263 (hand) during business hours. We require at least 48 hours advance notice of any schedule changes.

We ask that new patients arrive 30 minutes prior to their scheduled appointment time to register and established patients 15 minutes prior to appointment time. Failing to arrive on time may result in your appointment being rescheduled for a later date.

When calling to schedule an appointment, please have the following information available:
  • Nature of the problem
  • Type of insurance you have
  • Whether you have X-rays or MRIs to bring
  • Name of referring physician
** YOUR DRIVER'S LICENSE AND INSURANCE CARDS ARE REQUIRED AT THE TIME OF YOUR FIRST APPOINTMENT **
Fees & Insurance

Your insurance co-payment is required at check-in. There will be a $10 charge added to your account if you fail to pay your co-pay at the time of your visit. Patients without insurance must pay for their office visit prior to being seen. For your convenience, we accept Visa, Mastercard, cash or personal checks. It is your responsibility to confirm that Dr. Devine is a participating provider with your insurance company. It is also your responsibility that all referrals are completed by your referring physician, if any are required.

Due to the large volume of patients we see, we are not always able to answer the phone in a timely manner. Please leave a message and your phone call will be returned shortly.

Consent for Use & Disclosure of Health Information

Please have available any legal documents (i.e.Court papers/POA) if applicable. Patients under the age of 18 (including boarding school students) are required to be accompanied by a parent or guardian. A copy of the parent/guardian driver's license is required at check-in. Written consent to treat must be given by a parent if they will not be participating in the visit. If you are a resident/patient of a nursing home or rehab facility, it is required to have a family member or facility representative attend the appointment with you.

We make every effort to be courteous and helpful in our dealings with patients. In return, we expect appropriate behavior from patients and their families in both personal and telephone contacts. Failure to observe this policy will result in your discharge from this practice.
A large portion of our practice involves treating trauma-related injuries, which requires additional time to facilitate a proper treatment plan and may increase your wait time. Please know it is our intention to deliver quality care to each and every patient. We appreciate your understanding and patience. We recommend that you bring a book with you to your visit.
